Futrell Pharmacy now offers COVID-19 vaccines! The vaccine will be given by appointment only. Appointments are scheduled to allow for social distancing inside the pharmacy. Please call the pharmacy location of your choice to make your appointment.

To allow your appointment to go as quick as possible and to decrease exposure to COVID you can fill out our online form or download the form, fill it out, and bring it with you the day of your appointment.

Remember the following when you come to the pharmacy:

1. Bring a copy of your medical insurance card or your driver's license if uninsured.
2. Wear a short sleeved shirt.
3. Please be prepared to wait 15-30 minutes after your vaccination in your car to monitor for any adverse effects.
